RE:
Waiver request submitted by Noli C. Guinigundo, Coroner / West Carroll Parish, regarding a $2500 late fee assessed for filing his 2019 Tier 2 annual pfd 387 days late.
Facts:
Dr. Guinigundo stated he resigned from office on June 28, 2019, and relocated to Orlando, Florida. His Notice of Delinquency was mailed to his old address at the Coroner's Office, but no one from the Coroner's Office forwarded his mail or contacted him regarding the letter. He was unaware he was delinquent until he received the LFO. Dr. Guinigundo respectfully asks the Ethics Board to waive his late fee assessment.
