RE:
Waiver request submitted by Sherry Shelvin Guidry, former member of the New Iberia City Council (Pop. 30,617) / District 5 / Iberia Parish, regarding a $175 late fee assessed for filing her 2020 Tier 3 annual pfd 7 days late.
Facts:
Ms. Guidry stated her term ended in 2020, after serving her community for 4 years. She was no longer an elected official in 2021 when the report was due, so she was unaware she needed to file a final PFD. During the Covid-19 Pandemic she lost her job, and with being unemployed she needs the money to purchase household items and groceries.
