Waiver request submitted by Karen Collins, a member of the Lafourche Parish Hospital Service District No. 1 d/b/a Lady of the Sea General Hospital, regarding a $1500 late fee assessed for filing her 2021 Hospital Disclosure Statement 318 days late.
Facts:
Karen Collins' sister, Kim Guidry, was hired due to shortage of staff to provide covid vaccines. Ms. Guidry worked with three other retired nurses at drive-thru clinics administering the vaccines. The clinic ceased operation on 8/27/2021; however, her official separation date was 2/26/22, 6 months after her last documented work date per hospital policy. Ms. Collins read the filing requirements incorrectly and thought you disclose for the previous calendar year. Therefore, she filed in December 2021 to disclose information for 2021.

Law:
La. R.S. 42:1119B(2)(b)(ii) provides that no later than January thirtieth of each year, any chief executive and any member of a board of a hospital service district or hospital public trust authority whose immediate family member enters into an initial recruiting contract with or is employed by the hospital service district or hospital public trust authority shall file a disclosure statement with the Board of Ethics stating the facts of such employment. Any person who fails to timely file a disclosure statement under this Item may be assessed a late fee of fifty dollars per day, not to exceed $1,500.
Recommendations:
Suspend all based on future compliance.
