Angella Mayfield Rosa filed a waiver request on behalf of Malcom Wascom. Ms. Rosa stated she would submit the completed Hospital Disclosure on behalf of the board members. However, soon after Mr. Wascom was appointed, she had a double mastectomy. Due to complications, she was out of the office for the entire 2021 calendar year. Once she returned to the office in January 2022, she realized Mr. Wascom's disclosure was not sent to the BOE. Ms. Rosa mailed his completed disclosure. She asks the Ethics Board to waive the fine as this was an oversight on her part.

La. R.S. 42:1119B(2)(b)(ii) provides that no later than January thirtieth of each year, any chief executive and any member of a board of a hospital service district or hospital public trust authority whose immediate family member enters into an initial recruiting contract with or is employed by the hospital service district or hospital public trust authority shall file a disclosure statement with the Board of Ethics stating the facts of such employment. Any person who fails to timely file a disclosure statement under this Item may be assessed a late fee of fifty dollars per day, not to exceed $1,500.
