Waiver request submitted by Toby Jean Aguillard, former member of the Lafayette Parish Communication Commission, regarding a $1500 late fee assessed for filing his 2019 Tier 2.1 annual pfd 343 days late, a $300 late fee assessed for filing his amended 2019 Tier 2.1 annual pfd 6 days late, and a $300 late fee assessed for filing his amended 2020 Tier 2.1 annual pfd 6 days late. |

Mr. Aguillard is very sorry for the late filing; however, he served in this position because he was appointed Lafayette City Chief of Police. When he resigned in 2020 as Chief of Police, he was no longer a member of this board. He also lost his very competent secretary who helped remind him of his filings. Also, he states he never received the NOD since it was mailed to the Lafayette City Police Department.
