Alejandra Guzman is requesting a late fee waiver for the Tier 2.1 PFD Amendment. This notice was sent to her address during the winter holidays, but due to her attendance of family and work commitments, she was unavailable to collect her mail. Once she was able to collect her mail, she responded to the request that same day. She states that during her service on the Sewerage and Water Board of New Orleans, she complied with all the Board of Ethics' requirements and never missed any prior deadlines. Ms. Guzman would like to share that since she was on a volunteer board without any financial compensation, this fee would represent a financial hardship for her. She has dedicated time as a community advocate by attending monthly board and committee meetings, preparing assignments, and taking required training. She considers her role as a board member an important way of exerting her civil duties. She appreciates the consideration of the Board of Ethics and will be in compliance with all future requirements.

La. R.S. 42:1124.2.1
requires financial disclosure for each member and designee of a board or commission which has the authority to expend, disburse or invest $10,000 or more of funds in a fiscal year on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.
La. R.S. 42:1124.4 provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.
