RE:
Waiver request submitted by John Stone "Chip" Coulter, former member of the Workers Compensation Second Injury Board, regarding a $1500 late fee assessed for filing his 2020 Tier 2.1 annual pfd 356 days late and regarding a $1500 late fee assessed for filing his 2021 Tier 2.1 annual pfd 121 days late.
Facts:
John S. "Chip" Coulter stated that he does not necessarily have "good cause", but he truly believed the report was filed in 2020. As to 2021, he can only say that he didn't think of the report, due to work and home obligations. He is embarrassed and extremely apologetic. Mr. Coulter has worked for the state since 1986 and has always tried to do the right thing and regrets that he did not do so with this matter. He completed the statement and is no longer a member of the Workers Compensation Second Injury Board. He plans to retire from the state at the end of this Administration and hopes to do so in good standing. Mr. Coulter states that if it appropriate, he would like to appear at the Board of Ethics hearing.
