Clint Cormier is submitting a waiver request on behalf of his father, Alvin Cormier, who has been a constable in St Martin Parish from 2002 through December 2020. His father is 84 years old and has been under a doctor's care since the fall of 2020 with an uncurable illness called Idiopathic pulmonary fibrosis. He was also diagnosed with memory loss around that time. Clint Cormier recently received his father's late fee assessment. His father explained that he has no recollection of ever being late on any reports dating back to 2008 when this law was implemented. He does not recall receiving a delinquency notice in September 2021. He argues that he was no longer elected in 2021 and did not know he was obligated to file. Mr. Clint Cormier researched and realized his father was mistaken, so he filed the report on behalf of his father and signed since he is no longer able to sign his name. He is asking for a waiver of the late fee based on the fact that his father has never filed late and it would create a financial hardship for his father.

La. R.S. 42:1124.2
requires each person holding a public office who represents a voting district having a population of 5,000 or more persons to file an annual personal financial disclosure statement on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.
La. R.S. 42:1124.4 provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.
