A delinquency notice dated 3/25/2022 was left for Mr. Maurice "Scooter" Keen by the postal service at the front entrance and was not signed. On April 14, 2022, he contacted a BOE representative to let it be known that the letter wasn't signed or dated. The letter stated that he filed a Tier 2.1 instead of the needed Tier 2 annual PFD. He mailed his disclosure on 4/20/2022 and was received on 4/22/2022. On 4/27/2022, staff called Mr. Keen and he was informed that the form needed to be amended for failing to properly disclose his income. Mr. Keen stated that he assumed that the conversation was in reference to the amendment. He never received anything in writing until 2/10/2023, the Late Fee Order. Mr. Keen intended to correct the Tier 2 statement. He assumed that it had been amended correctly. He would like to ask for a waiver of the late fee.

La. R.S. 42:1124.2
requires each person holding a public office who represents a voting district having a population of 5,000 or more persons to file an annual personal financial disclosure statement on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.
La. R.S. 42:1124.4
of the Code provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.
