Rowena Cherie Russel Lipsey served as a member of the Board of Commissioners for Concordia Parish Hospital Service District Number One d/b/a Trinity Medical in Ferriday, LA from April 2017 to the fall of 2022. Her daughter, Brandi Lipsey, and daughter-in-law, Jami Lipsey, married on 11/11/2022. Brandi was hired on an as-needed basis(PRN) on 11/16/2022. When Brandi and Jami resigned, Ms. Lipsey did not remember to file. On Jami's last day 2/13/2023,Ms.Lipsey realized that Jami and Brandi had worked past the 1/30/23 deadline. She discussed this with the Chief Compliance Officer Walton Wilson and he assisted her with filing the disclosure. She is a retiree who lives on a fixed income and paying this fine would be a financial hardship. Ms. Lipsey assures that moving forward she will be sure to comply. She asks that you waive the assessed late fee.

La. R.S. 42:1119B(2)(b)(ii)
provides that no later than January
thirtieth of each year, any chief executive and any member of a board of a
hospital service district or hospital public trust authority whose immediate
family member enters into an initial recruiting contract with or is employed by
the hospital service district or hospital public trust authority shall file a
disclosure statement with the Board of Ethics stating the facts of such
employment. Any person who fails to timely file a disclosure statement under
this Item may be assessed a late fee of fifty dollars per day, not to exceed
$1,500.
