Waiver request submitted by Brenda K. Goode, Pointe Coupee Hospital Service District No.1, regarding a $1500 late fee assessed for filing her 2023 Hospital Disclosure 109 days late. |
|
Brenda K. Goode was appointed to the Pointe Coupee Health District No. 1 Board in January 2022 and submitted her disclosure, but was unaware that it should be submitted annually. In November 2022, she received a reminder to complete the BOE training. At the January board meeting, she received the packet containing the disclosure but believed that she didn't have to resubmit since she held a term position. Had she known to file the report she would have complied as she has previously. When she received the notice she was leaving to help her daughter in Florida with her three sons. Ms. Goode would like to ask that the fee be waived or reduced.
