Waiver request submitted by Jodi Jacobs Aamodt, former member of the KIPP New Orleans Charter School, regarding a $1500 late fee assessed for filing her 2019 Tier 3 annual pfd 99 days late.
Facts:
Jodi Jacobs Aamodt no longer has records from when she served on the board of KIPP New Orleans but to the best of her recollection, she rotated off that Board at the beginning of 2019. When she received the notice she was operating under the assumption that it was sent in error because she believed she was not on the Board in 2019. Once she realized that she was on the Board for a short period of time she sent the report in late. Whatever correspondence was sent out regarding this matter was sent in 2020. During the pandemic, her office temporarily shut down and her husband was laid off. Ms. Aamodt was the primary caretaker for her mother who suffered from dementia and ultimately passed away on 01/19/2022. She is also in the process of shutting down her office. She was behind on several important things that she had to put back on track. Ms. Aamodt would like to humbly ask that you waive the late fee.

Law:
La. R.S. 42:1124.3 requires persons holding a public office who represent a voting district having a population of less than 5,000 and each member of the governing authority or management board of a charter school created pursuant to Chapter 42 of Title 17 of the Louisiana Revised Statutes of 1950 to file a financial disclosure statement on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.

La. R.S. 42:1124.4 provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.
