Eugene P. Foulcard is submitting a waiver request and states the following:
Due to unforeseen circumstances, Mr. Eugene Foulcard filed his report late. After waiting for receipts to arrive and misplacing them, he needed to request them again. The wait caused a delay in filing his report but allowed him to file it correctly. This does not happen often, so he hopes to be granted a late fee waiver. Mr. Foulcard does not wish to appear before the Board, but he views this as a learning experience and assures all reports will be submitted on time.
