Mildred E. Methvin was appointed to the Board of Supervisors for the University of Louisiana System, representing the 3rd Congressional District, effective 1/1/2019 until 12/31/2024. Six weeks after being appointed, her partner suffered a stroke. Ms. Methvin commuted between Lafayette and New Orleans for over two years to oversee her partner's medical care. In the fall of 2020, she purchased a home in New Orleans and was an active member of the ULL System Board. In the spring of 2022, she sold her initial home. On 2/2/2022, Ms. Methvin sent a letter of resignation explaining she would no longer live in the 3rd Congressional District, which she was supposed to represent on the Board. On 5/6/2024, she received a reminder to submit her disclosure. She received an email from the Board assistant to verify completion but did not receive a reminder to file for the 2022 calendar year. Ms. Methvin relocated to Minneapolis in 2/2024 and sold her New Orleans home. She is sure she did not receive the Notice of Delinquency (NOD) on 4/15/2024 but is unsure of the exact date. On 7/15/2024, she received guidance on how to file a waiver.

La. R.S. 42:1124.2.1 requires financial disclosure for each member and designee of a board or commission which has the authority to expend, disburse or invest $10,000 or more of funds in a fiscal year on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.
La. R.S. 42:1124.4 provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.
