Mr. Switzer reasserts financial hardship as well as medical issues; however, he has failed to respond to requests for supporting documentation.
Initial Waiver Request
Dennis L. Switzer has done his best to complete his reports. He went through the same process in earlier years and paid a $1,500 fee. He was unaware that he needed to complete the disclosures when he accepted the appointment to the Lafourche Parish Water District No. 1 since the position doesn't pay a fee to attend their regular meetings. Mr. Switzer has retired and is living on a fixed income. He was in lockdown mode for a over a year due to the COVID epidemic and later suffered $47K worth of damage due the Hurricane Ida. On May 25, 2022, he was diagnosed with complete kidney failure. He has been having dialysis treatment three days a week- which will continue- and they cost approximately $900 per month after Medicare goes into effect. He still has $16K in hurricane damages that were not covered by his insurance.

La. R.S. 42:1124.2.1 requires financial disclosure for each member and designee of a board or commission which has the authority to expend, disburse or invest $10,000 or more of funds in a fiscal year on or before May 15th of each year during which the person holds office and the year following termination of the holding of such office.
La. R.S. 42:1124.4 provides that upon discovery of the failure to file, failure to timely file, omission of information or inaccurate information, the Board shall notify the filer by sending him a notice of delinquency by certified mail. The notice of delinquency shall inform the person that the financial statement must be filed, or that the information must be disclosed or accurately disclosed, or that a written answer contesting the allegation of such failure, omission, or inaccuracy must be filed no later than seven (7) business days after receipt of the notice of delinquency.
