Waiver request submitted by Davondra Brown, former member of the Louisiana Commission for the Deaf / Designee, regarding a $1500 late fee assessed for filing her 2022 Tier 2.1 annual pfd 26 days late and a $1500 late fee assessed for filing her 2023 Tier 2.1 annual pfd 26 days late. |

Davondra Brown was not notified of this requirement by the Louisiana Department of Health (LDH) or the Commission for the Deaf while formally serving as Director of the Bureau of Community Partnerships and Healthy Equity. She vacated the seat with LDH in August 2023. Ms. Brown received the first notice 6 months after the statute's expected filing date for the 2023 appointment and 18 months after the 2022 filing date. After receiving the FTF notice, she experienced multiple medical issues. Ms. Brown believes being given less than 10 days to amend is unethical.
