Waiver request submitted by Melanie M. Washington, former member of the Children's Trust Fund Board, regarding a $950 late fee assessed for filing her 2022 Tier 2.1 annual pfd 19 days late. |

When Melanie M. Washington received the NOD, she was not in a good mental space. She recalls Sherry Lassere reaching out to her to get her email address but does not know when she read the document. Ms. Washington has been battling depression for a few months. She was unable to secure employment after leaving the Governor's office on 1/8/2024 and does not have a spouse to support her financially. Ms. Washington was not eligible for unemployment and has a mortgage in addition to several bills. She had to let go of some responsibilities in organizations due to her mental health. Ms. Washington started a new position on 8/16/24 with low pay, and she is unable to afford another unexpected bill after being unemployed for eight months. She acknowledges this entire situation is due to her negligence.
