Jennifer S. Greenleaf, report preparer for Altice USA Louisiana PAC, is requesting a waiver and states based on the records and requirements for filing the 40-G, the PAC was not required to file. They did not participate in the municipal general election because there were no contributions or expenditures. The PAC only contributed to the Louisiana Republic Legislative Delegation Campaign Committee during the reporting period and based on the public record they did not participate in the municipal general election. In mid September, Ms Greenleaf extended medical leave for surgery and recovery associated with a previous medical issues. While on leave, she was advised by a BOE representative to amend the report. When she returned to the office, she received the LFO. Supporting documents have been submitted.
