Dr. Tia Mills requests a waiver and states the following: It was her first time serving in an office of that magnitude, and she did her best to ensure all deadlines were met. Having limited knowledge of report filing, Dr. Mills made every effort to seek guidance from the BOE after receiving her delinquency notice. However, she had issues making contact. Since this incident, all required reports have been filed timely. Dr. Mills humbly asks that this penalty be waived. She would like to thank you in advance for considering his matter.
