Mr. Duncan states that his family went on vacation September 20th until October 6th. He received the email saying the report was due and filled out the information, however, he did not check the "I agree" box but hit the finalize and file button. He also states that he did not wait to see if it filed and never checked his email to see if he received a confirmation. He admits fault but was in a hurry. Mr. Duncan is requesting a waiver or reduction in the fine.
