| Waiver request submitted by Marty Andrew Campbell, former member of the Southside Economic Development District, regarding a $1500 late fee assessed for filing his amended 2021 Tier 2.1 annual pfd 157 days late, and;a $1500 late fee assessed for failing to file his 2023 Tier 2.1 annual pfd. |

Marty Campbell stated he served on the Southside Economic Development Board in Monroe, LA, from 2021 to 2023. He mistakenly submitted the incorrect disclosure. Mr. Campbell has worked in healthcare for more than 20 years. His job requires him to travel and be away from home for weeks or even months. The forms may have been misplaced. Mr. Campbell did not intentionally ignore any requests. He would like to apologize for this mishap and request a waiver of the assessed late fee.
In another letter faxed on 10/8/2025 to the BOE, Mr. Campbell stated that he did not serve on the SEDD Board in 2023, as he verbally resigned on 12/12/2022.
*On 10/13/2025, Disclosure Director, Ashley M. Wimberley, spoke with the former SEDD Chairman, Tony Little, who stated that Mr. Campbell did not resign. Mr. Little then emailed stating that Mr. Campbell verbally resigned on 12/12/2022. This information conflicts with the term information presented in Mr. Campbell's initial Waiver Request faxed 3/17/2025, the 2023 LLA Audit Report, and New Member Appointment Resolution from the Monroe City Council.
