Willie Breaux did not get notice from the superintendent's administrative assistant that the disclosure was due. The administrative assistant is responsible for keeping up with and disseminating all material to the school board members. She was promoted to a new position, and her replacement had no prior experience. In addition, the post office did not deliver the NOD on time. Ms. Breaux was home when the notice was delivered, but it was left near the front door instead of the mailbox. She immediately faxed the disclosure after receiving the notice. This happened with the LFO as well. Ms. Breaux takes her position seriously and would like to apologize for the mishap. Please waive the assessed late fee.

La. R.S. 42:1119B(2)(a)(iii) by September fifteenth of each school year, any school board member or superintendent whose immediate family member is employed by the school board shall file a disclosure statement with the Board of Ethics stating the facts of such employment. Any person who fails to timely file a disclosure statement under this Item may be assessed a late fee of fifty dollars per day, not to exceed one thousand five hundred dollars, subject to the provisions of R.S. 42:1157.
