Jay H. Banks requests a waiver and states the following: He states there was no intent not to comply. He was unaware he needed to file since the account was zeroed out. Mr. Banks ran for public office with the intention of helping his community and does not regret serving because he made a difference. Mr. Banks is not wealthy, but they invested money to run the campaign. The campaign was closed out with all debts paid except the one owed to him. The personal loan he made to his campaign has been forgiven. Mr. Banks appreciates your consideration in waiving the assessed late fee.
