RE:
Waiver request submitted by Stephen Wayne "Steve" Prator, Board of Pardons / Caddo Parish Sheriff, regarding a $300 late fee assessed for filing his amended 2024 Tier 2 annual pfd 3 days late.
Facts:
Stephen Prator timely filed the Boards of Pardons and Committee on Parole PFD, but forgot that he served as sheriff for two months in 2024, which required a different form. He retired before the end of his term to accept the Pardon Board appointment, which he admits was a mistake. While leaving for vacation, he received the amendment notice. He completed the form and intended to fax it on 10/17/2025, but the cost would have been $50.00. So, he faxed it on 10/20/25 upon his return to Louisiana, one business day late. He did not have access to a computer to upload it. Mr. Prator has amended only a few disclosures during his 17 years of service. Please waive the assessed late fee.
