| Waiver request submitted by Mr. Donald W. "Don Smith, former Coroner / Bienville Parish, regarding a $600 late fee assessed for filing his amended 2024 Tier 2 annual pfd 6 days late. |

Donald W. Smith's term as coroner concluded on 3/25/2024, and he is no longer considered an elected official, which he understands relieves him of further financial reporting obligations after 2024. Mr. Smith retired to care for his wife during her prolonged illness, who passed away on 3/16/2025. Her passing resulted in profound emotional distress and ongoing financial hardship. Much of his savings was depleted. These circumstances have made it difficult to fulfill administrative requirements in a timely manner.
