STATE OF LOUISIANA <br /> DIVISION OF ADMINISTRATIVE LAW <br /> LOUISIANA BOARD OF ETHICS <br /> DOCKET NO. <br /> IN THE MATTER OF <br /> AGENCY TRACKING NO. 2017-672 <br /> MICHELLE DENNIS <br /> CHARGES <br /> The Louisiana Board of Ethics voted on June 15, 2018 to and does hereby file the <br /> following charge: <br /> 1. <br /> Michelle Dennis served as a clerk for the Fifth Ward Elementary School, within the St. <br /> John the Baptist School System, from August 10, 2009 until February 15, 2017. <br /> 2, <br /> Michelle Dennis' duties as clerk included preparing purchase orders, required <br /> requisitions, maintaining office supplies, maintaining fiscal accounts and related financial record <br /> keeping and reporting duties. Additionally, she was required to file and maintain the necessary <br /> corresponding deposits, statements, cash handling forms, and related St. John the Baptist School <br /> Board money tracking forms. <br /> 3, <br /> Michelle Dennis used the Fifth Ward Elementary School Sam's Club credit card, of <br /> which she had custody and control, to make sixteen, separate personal purchases totaling one <br /> 1 <br />
