LOUISIANA BOARD OF ETHICS <br /> IN THE MATTER OF <br /> HAYWOOD FAIR DOCKET NO: 2012-1647 <br /> CONSENT ORDER <br /> The Louisiana Board of Ethics(the"Board"),pursuant to the authority contained in La.R.S. <br /> 42:1141, filed charges against Haywood Fair. <br /> I. <br /> Madison Parish Hospital Service District owns and operates Madison Parish Hospital. The <br /> board of directors are appointed by the Madison Parish Police Jury. <br /> II. <br /> Hayward Fair was a member of the Madison Parish Hospital Service District's board of <br /> commissioners from 2008 to August 2012. <br /> III. <br /> The Madison Parish Hospital Service District purchased a tractor with a bush hog attachment <br /> in August 2008 to be used for the Madison Parish Hospital Service District business only. <br /> IV. <br /> Mr. Fair used the Madison Parish Hospital Service District's tractor to cut grass on private <br /> property when the grass in question was "too high" for him to cut using his personal lawn mower. <br /> V. <br /> Mr. Fair was issued a credit card by the Madison Parish Hospital to be used for official <br /> Madison Parish Hospital business. <br /> Page 1 of 5 <br />
