LVFC uses equipment and fire trucks owned and maintained by Fire District 2,and Fire <br /> District 2 pays LVFC a monthly"rent"of$1,200.00 to store its equipment at LVFC's six stations. <br /> Fire District 2 holds insurance policies on all LVFC's properties and buildings. <br /> 4. <br /> Armand Autin, Jr. ("Mr. Autin") has been a member of LVFC since 1992. His wife, <br /> Nichole Autin ("Mrs. Autin"), has been a volunteer member of LVFC for over 27 years. Since <br /> October 1, 2012, Mr. Autin has continuously served as Fire Chief of LVFC. <br /> 5. <br /> As Fire Chief, Mr. Autin is responsible for ensuring all LVFC volunteers are trained and <br /> competent to respond to calls, ensuring all equipment is operating properly, coordinating with <br /> parish government on emergency preparedness, managing volunteer personnel, and allocating <br /> resources during calls. <br /> 6. <br /> On June 26,2012,Mr.and Mrs.Autin registered the company Baja Enterprises of Lockport <br /> LLC ("Baja") with the Louisiana Secretary of State. Mr. Autin and Mrs. Autin each own a 50% <br /> interest in Baja. Prior to registering Baja with the Secretary of State, Mrs. Autin operated the <br /> business under the name "Just Do It" and provided lawn care services. Baja/Just Do It has no <br /> einployees. Mrs.Autin performs the lawn care services and manages the business operations while <br /> Mr. Autin maintains the lawn equipment. <br /> 7. <br /> BajalJust Do It provided lawn care services to LVFC's six fire stations at a rate of$400.00 <br /> per month, which covers gas for travel to the stations, gas for lawn cutting, and maintenance on <br /> the equipment. There are no written contracts between BajalJust Do It and LVFC related to lawn <br /> __ _ _ <br />
