"TED^CALLOWAY <br /> The Board of Ethics (the "BOE"), pursuant to the authority contained in the Louisiana <br /> Code of Governmental Ethics(hereinafter referred to as the"Ethics Code"),La. R.S. 42:1101, et. <br /> seq., and particularly Section 1141 of the Ethics Code, conducted a confidential investigation <br /> concerning information that Edward E. "Ted" Calloway, in his capacity as an employee of the <br /> G.B. Cooley Hospital Service District, may have violated Sections 1111A(1)(a) and 1112(A) of <br /> the Ethics Code by virtue of his receipt of money from Hospital District accounts. <br /> On the basis of information obtained during the investigation, the BOE and Edward <br /> Calloway enter into this Consent Opinion as follows: <br /> I. PARTIES: <br /> The Parties hereto are: <br /> Edward "Ted" Calloway, a person of the full age of majority, domiciled in the <br /> Parish of Ouachita, with a last known address of 201 Briarcliff Drive, West <br /> Monroe, Louisiana, 71291. <br /> The Louisiana Board of Ethics, an agency of the State of Louisiana, whose <br /> mailing address is P.O. Box 4368, Baton Rouge, Louisiana, 70821. <br /> H. JURISDICTION: <br /> 1. <br /> The BOE is an agency of the State of Louisiana domiciled in East Baton Rouge Parish, <br /> Louisiana. <br />
