Miscellaneous Forms
Ethics Agent Form	Form to become an Ethics Agent pursuant to La. R.S. 42:1170C(4). Please click here to register and create your Ethics Education Liaison profile if you have not already done so. After you create your profile and are logged in, please select “Create a New Training Report/Registration” and complete the LiaisonReg form, which must be signed by the head of your agency.

Electronic Filing Affidavits

In accordance with Section 1801 B of the Rules of the Louisiana Board of Ethics, Electronic Filing Affidavits should be received by our office at least five business days before a filing deadline.

If the affidavit is received at least 5 days ahead of the deadline, the password and ID will be issued in order to file that report electronically.

Affidavits received less than 5 days ahead of a deadline will be processed as quickly as possible.

Candidate Electronic Filing Affidavit	Electronic filing affidavit for those using the LEADERS software (Candidates). Candidate Committees should also use this form. Mail your notarized affidavit to the Louisiana Board of Ethics. Include a note with the name and phone number of the person who will be filing the reports electronically and a member of the Technical Support Staff will contact them with their password. You may also contact us to request that the affidavits be mailed to you. If you would also like a users guide, please indicate Candidate.
Disclosures Electronic Filing Affidavit	Electronic filing affidavit for using the PFD E-Filing system. Mail your notarized affidavit to the Louisiana Board of Ethics. If you already have a username and password for electronically filing campaign finance reports through Leaders Online, the same username and password can be used to log in and access the personal financial disclosure forms on the PFD E-file system.
Other Persons Electronic Filing Affidavit	Electronic filing affidavit for "Other Person" filers. Mail your notarized affidavit to the Louisiana Board of Ethics. Include a note with the name and phone number of the person who will be filing the reports electronically and a member of the Technical Support Staff will contact them with their password. You may also contact us to request that the affidavits be mailed to you.
PAC Electronic Filing Affidavit	Electronic filing affidavit for Political Committees (such as PACs) filers. Mail your notarized affidavit to the Louisiana Board of Ethics. Include a note with the name and phone number of the person who will be filing the reports electronically and a member of the Technical Support Staff will contact them with their password. You may also contact us to request that the affidavits be mailed to you. If you would also like a users guide, please indicate PAC.
